Emergency Contact Information – Who do you want contacted in case you are involved in a medical emergency? Select and store the information on the trusted people you want to be contacted in case of an emergency. You can even prioritize them in a calling order and show what state they are in incase of a regional disaster like Katrina. This information is available in both summary and detail formats.

Personal Emergency Card – How do you answer important questions when you are too sick or injured? With the Emergency Wallet Card designed for easy printing with vital information that first responders can use in the event of an emergency. This card has been designed to incorporate the important information - Who to Call, Current Medications, Known Allergies and Physician Contact Information. This card also contains information required to access your Crisis ID Emergency Medical Report. Updating your wallet card whenever new information is loaded ensures that you have the most current information at your disposal. It is suggested that you keep the most current copy of your Wallet Card next to your primary form of identification, driver’s license or school identification.

Emergency Medical Report – You have complete control over the information that you want disclosed in your reports. By selecting the “Setup CrisisID report” link from the Member Home Page you control what information the Emergency Medical Report (EMR), a read only report, contains.
